Abstract
This Regulation, made under Section 19 (2) of the Veterinary Act, establishes precise requirements and procedures for keeping records and submitting reports and data on the provision of veterinary services, including: cases of illness and animal diseases, performed treatment and other procedures, medicines and medicated feed prescribed on the basis of a veterinary prescription; use of medicines during the provision of veterinary services. Matters covered by this Regulation concern: data included in the record of the provision of veterinary services, including: the name and the quantity of the medicine used during the provision of veterinary services, its package code; report on diagnosed animal diseases; report on vaccinations performed; specific data to be provided by the veterinarian on prescribed drugs and drugs issued and used during the provision of veterinary services, and detailed procedure for data submission; animal species, categories and age groups, including: camelids, deer, poultry, aquaculture animals.
