Abstract
This Regulation determines the procedure for issuing veterinary (health) certificates for animals and products of animal origin. In order to receive a veterinary (health) certificate for the shipment, the operator submits an application to the service's territorial unit, prepared by filling out an online form on the service's e-service website or in a free form. In the application form the following information must be indicated: (a) sender and recipient (first and last name or name, address, country); (b) place of dispatch (name, if any, address of actual location, registration or confirmation number assigned by the service, if any); (c) place of loading (name, if any, address of the actual location, registration or confirmation number assigned by the service, if any), date and time; (d) place of receipt (name, if any, address of the actual location, registration or approval number assigned by the competent authority, if any); (e) transit country, if the shipment is intended to be transported in transit; (f) the vehicle intended for transporting the consignment (type, registration number, number of the permit granted by the competent authority to the vehicle for the transport of animals - if the consignment contains live animals; (g) data of the carrier (first and last name or title, number of the permit granted by the competent authority for the permit of the animal carrier - if the shipment contains live animals); (h) the planned duration of transportation and the area allocated during transportation, if the shipment contains live animals; (i) description of the shipment (for products of animal origin – type, company where the product is produced, number of packages (if applicable) and weight; for animals – species, category, identification method and number, age, sex, quantity, number of packages (if applicable) and weight); and (j) the purpose for which animals or animal products are moved.
