Regulation on the application form for qualification in the public procurement procedure.

This Regulation prescribes the application form for qualification in the public procurement procedure. The application for qualification in the public procurement procedure contains data on: 1) the date and time of submission of the application for qualification; 2) status of application for qualification; 3) to the applicant for qualification; 4) public procurement, namely: - the name of the ordering party, - the procedure code, - the description of the subject of the public procurement, - the type of subject, - the type of procedure, - the type of phase; 5) conditions for participation in the procedure.
