Regulation concerning the record on controlled substances.

The present Regulation of the Minister of Economy, Labour and Social Policy, which is composed of seven paragraphs and four Annexes, establishes: the way of keeping records on controlled substances; the methods to exchange information contained in the records; forms of questionnaires of the record. The forms of the questionnaires are specified in Annexes 1 to 3. Annex 4 specifies the categories of origin of controlled substances.
