Regulation on form of transport document for sheep and goats.

The Regulation of the Minister of Agriculture and Rural Development is composed of two paragraphs and one Annex. The form of the transport document for sheep and goats is specified in the Annex. The form includes the following information: date of leaving the herd; herd base identifier; first name and surname of the owner; address of the owner; number of transported animals; identifiers of transported animals; place of destination of animals; make and registration number of the vehicle; veterinary identifier of the hauler; identifier of the certificate issued by a veterinarian, permitting the vehicle to transport animals; signature of the animals owner.
