Abstract
The Slovenian National Diabetes Management Program for the period form the year 2020 until the year 2030 is a development strategy for the next decade which defines three overarching goals: 1. to strengthen the health of the population with an emphasis on diabetes with activities that are complementary to other programs, plans and projects: − by ensuring the conditions for a healthy lifestyle and strengthening health in Slovenia in the area of risk factors for type 2 diabetes; − by raising public awareness of diabetes; − by supporting communities (family, local community, work organization, region, country) to actively co-create an environment in which better health is possible for everyone, especially individuals with unfavorable determinants of health; − by supporting the empowerment of individuals for a healthy lifestyle and by strengthening health literacy and education by ensuring the availability of programs and other health-enhancing measures for all groups of the population and by reducing the burden of chronic diseases with special emphasis on at-risk and vulnerable groups; 2. postpone or prevent type 2 diabetes and increase the chances of its early detection in individuals by ensuring timely recognition and structured care, in which action is proportional to the level of risk of developing type 2 diabetes, and ensuring empowerment and a high level of health literacy, specially adapted to individuals with unfavorable health determinants in: − persons with risk factors for type 2 diabetes; - persons with borderline basal glycemia or impaired glucose tolerance; − children, adolescents and pregnant women; 3. reduce complications and mortality from diabetes and increase the quality of life, including long-term care: − with access to coordinated, connected, integrated, continuous, lifelong, effective, safe and quality care that actively involves the person with diabetes in decision-making and is based on adequate organization of care and monitoring of its quality; − by providing conditions for strengthening health literacy and supporting the empowerment process for successful self-management of diabetes; − with continuous professional upgrading of the knowledge and skills of experts working in healthcare; − through training and appropriate organization of activities of interested persons outside the healthcare system, who can significantly contribute to the care of people with diabetes through their activities.
