Decree No. 896 of the Cabinet of Ministers validating Licensing terms and conditions for performing veterinary practice.

These Licensing terms and conditions are developed in accordance with the Law of Ukraine "On Licensing Types of Economic Activities" and define an exhaustive list of requirements that are mandatory for the licensee, as well as an exhaustive list of documents attached to the application for obtaining a license for conducting business activities related to veterinary practice. Economic activities related to veterinary practice can be performed on the whole territory of Ukraine by legal entities as well as natural persons – individual entrepreneurs - veterinary medicine specialists in accordance with a license, in the presence of the material and technical base required for the conduct of the corresponding type of activity, and subject to compliance of such persons with qualification, organizational, personnel and technological requirements stipulated by these Licensing terms and conditions. The following documents must be attached to the application for obtaining a license: (a) information on the material and technical basis of the business entity required for the conduct of business activities related to veterinary practice (Annex 2); (b) information of the business entity on the presence of specialists having the educational and qualification level necessary for the conduct of economic activity related to veterinary practice (Annex 3); (c) certified copies of documents confirming the educational and qualification level of the specialists required for conducting the corresponding type of economic activity; (d) a copy of the passport of the licensee (authorized representative thereof) with the mark of the controlling body about the notice of refusal due to his religious beliefs from the adoption of the registration number of the taxpayer's registration card (submitted only by natural persons - entrepreneurs who, because of their religious beliefs, refused to accept the registration number of the registration card of the taxpayer and notified the controlling authority); and (e) description of documents submitted for obtaining a license, in duplicate (Annex 4).
