Order No. 109 of the Ministry of Public Health validating the unified form of the act, consisting of the results of the planned (unscheduled) measures of state supervision (control) regarding compliance by the subject of the requirements of the licensing conditions for performance of economic activities for the cultivation of plants included in Table I of the list of narcotic drugs, psychotropic substances and precursors, approved by the Cabinet of Ministers of Ukraine, design, manufacture, production, storage, transportation, purchase, sale (release), import into the territory of Ukraine, export from the territory of Ukraine, the use, destruction of narcotic drugs, psychotropic substances and precursors included in the list, and the form of the administrative document.

This Order validates the form of Report drawn up on the basis of the results of a planned (unscheduled) measure of state supervision (control) regarding the compliance by the subject of the licensed conditions for performance of economic activities for the cultivation of plants included in Table I of the list of narcotic drugs, psychotropic substances and precursors (Annex 1) and the form of Report on the basis of which a decision is taken on cancellation of the license granting the right to perform economic activities for the cultivation of plants included in Table I of the list of narcotic drugs, psychotropic substances and precursors (Annex 2).
