Regulation on the form and content of the request for expert control of the production of planting material and the form and content of the certificate of quality of planting material.

This Regulation determines the form and prescribes the necessary contents of the request for expert control of the production of planting material and the form and content of the certificate of quality for planting material (forest tree species reproductive materials). The request shall contain: name of authorized institution, name of legal entity, address and headquarters, municipality, name and location of the seed nursery for planting material whose production is in progress, wood type, quantity, age, category, number of the certificate of origin and method or method of production (classification/container), signature of the requester and a place for the stamp or label. The form of the request is given in Annex 1, which is an integral part of this text.
