Decree No. 808 of the Cabinet of Ministers validating Licensing terms and conditions for economic activities related to manufacturing of veterinary drugs.

Licensee is required to comply with the requirements of these Licensing terms and conditions, and the applicant for a license for the manufacturing of veterinary drugs for obtaining it shall comply with these Licensing terms and conditions. Performance of economic activity for the manufacturing of veterinary drugs is carried out through a production site, warehouse area (storage space, warehouse), zone of quality control, and zone for issuing a permit for the release of veterinary drugs. The requirements of these Licensing terms and conditions apply to the manufacturing site of the licensee, which are indicated in the documents attached to the application for obtaining a license. In order to obtain a license for the manufacturing of veterinary drugs, the business entity submits to the licensing authority an application for obtaining a license (Annex 1). The application shall indicate all the location of the manufacturing facility of the licensed business activity. The following documents are attached to the application for obtaining a license: (a) supporting documents for each manufacturing facility of business activity for the production of veterinary preparations: information on the availability of the material and technical base and availability of specialists having the educational and qualification level necessary for conduct of economic activity for the production of veterinary drugs (Annex 2); a copy of the dossier of the veterinary medicine manufacturer's section approved by the entity; (b) a copy of the passport of the head of the applicant of the license (his authorized representative) with the mark of the relevant supervisory authority on the refusal due to his religious beliefs from the adoption of the registration number of the tax card's account holder (only provided by natural persons - entrepreneurs who, because of their religious beliefs, refuse to accept the registration number of the accounting taxpayer cards and informed the controlling authority thereof); (c) information on the lack of control (in the meaning of Article 1 of the Law of Ukraine "On the Protection of Economic Competition") for the activity of the applicant for a license of persons - residents of other states that carry out armed aggression against Ukraine (in the sense referred to in Article 1 of the Law of Ukraine "On defense of Ukraine ") and/or whose actions are created conditions for the emergence of a military conflict and the use of military force against Ukraine. Such information is provided in an arbitrary form; and (d) description of documents submitted for obtaining a license, in duplicate (Annex 3).
