Regulation on the form and content of the document on origin of stocking material and the manner of its issuance and completion.

This Regulation determines the form and prescribes all necessary contents of the document on the origin of stocking materials (see attached form in the Annex of this text). This text also sets the method of its issuance and completion. The document on the origin of the stocking material intended for trade shall contain specific data as stated in article 2 (including name and address of the headquarters of reproduction center or fish farm, fishing water in which the fish was located and details on the facility for the production of deriving fish originated consumable materials).
