Regulation on form and content of the form the certificate of origin of game, game parts and game trophies, as well as method of keeping records on the owner and origin of game and game parts.

This Regulation determines the form and defines the necessary contents of the form that certifies the origin of game, game parts and game trophies. This text also sets the method for keeping records on owners and related origin of hunting game and hunting game deriving parts. In the form of the certificate of origin of game, game parts and game trophies (all give in the Annex of this text), the following data has to be entered: name of the hunting ground, purpose of the hunting ground, concessionaire of the game in the hunting ground, serial number, order number, species of live game, shot game and parts of game, sex, number of heads shot, trophy value of the game, note, first and last name of the hunter, place of stamp and first and last name of the responsible person.
